8/14/2023 EnchantaVaughn Approved Behavior: It was noticed by QMHP-C, Ben-Nita had a tired look on her face but switched moods once session started. Ben-Nitaâs energy increased once she got comfortable in the home, initiating a conversation with QMHP-C. Behavior: Ben-Nita did not have to be directed on sharing or waiting her turn, and QMHP-C praised her for her excellent behaviors going into the second half of the session. Ben-Nita was receptive to the idea of practicing breathing technique and meditation strategies. Ben-Nita was cooperative and appeared to be learning a lot from todayâs session. Intervention: QMHP-C inquired about morning experiences during initial in-home contact with Ben-Nita. QMHP-C asked how Ben-Nita was feeling and if she needed anything. QMHP-C will review problem solving and give feedback regarding ADHD and its causes and effect to well-being and mental performance. QMHP-C encouraged Ben-Nita to be receptive to all feedback and to take in what is being said rather than how itâs being said. QMHP-C will teach Ben-Nita meditation skills to decrease any mental or emotional tension. QMHP-C assisted Ben-Nita in developing more communication skills. Intervention: QMHP-C assisted Ben-Nita in normalizing the changes that are occurring in the home and outside the home. QMHP-C reflected with Ben-Nita on her choices, reviewing both good and bad experiences. QMHP-C processed with Ben-Nita, "how can you add more value to your life?â. QMHP-C modeled pro-social behaviors for Ben-Nita to strengthen her understanding of what to do in the program both in person and virtually, as well as, at home and out in the community. QMHP-C encouraged Ben-Nita to have a good evening upon ending the session. Response: Ben-Nita transitions smoothly and safely to session area, displaying appropriate behaviors. Ben-Nita was seen waiting for instruction. Ben-Nita understands that problem solving starts with thinking before acting. Ben-Nita demonstrates problem solving by staying calm and using her senses. Ben-Nita acknowledges her improvements and vows to stay consistent with her therapy. Ben-Nita have been doing a better job controlling impulsive and hyperactive behaviors, due to keeping her hands to herself and staying in designated area. Response: Ben-Nita exhibits appropriate behavior in the home, as observed during the role play activity. Ben-Nita displays good participation and displayed good interaction with her family, acknowledging their love and support for her session time. Ben-Nita understands that sessions are a privilege and she have to show good work ethic and follow along with the interventions, in order to maintain his ISP objectives. Ben-Nita continues to make progress in the home, as reported and observed. Ben-Nita did an excellent job turning her day around and reaching her goals.
